TC for PMTCT Support Tools

Early diagnosis of HIV in infants provides a critical opportunity to strengthen follow-up of HIV-exposed children, assure early access to ARV treatment for infected children, provide reassuring information to families of uninfected children, aid evaluation of PMTCT programs, and increase capacity to use laboratory technology. In response to a need for standardized guidance for early infant diagnosis (EID) of HIV, 3 manuals were developed and are available below:

As part of the President's Emergency Plan for AIDS Relief (PEPFAR), the CDC Global AIDS Program and laboratory branches that support PMTCT programs in resource-limited countries have developed these manuals to support quality early infant diagnosis programs and allow rapid scale-up of infant diagnosis and treatment. These resources give an overview of how to set up infant diagnosis services. In particular, they suggest strategies for early identification of HIV-exposed and infected infants within the context of well-child care, and promote HIV testing of children presenting to health care settings with signs and symptoms of HIV. As part of this effort, the books address how to train clinic staff and develop laboratory infrastructure within resource-limited countries to support early infant diagnosis services. Guidance on designing testing algorithms is provided, taking into account age, clinical presentation and local infant feeding guidelines. The package also includes tools: logistical planning tools and calculators, laboratory guidelines, forms, logbooks, and other materials for use in infant diagnosis programs.

Content Review and Regional Adaptations

These manuals were reviewed as part of a workshop in May 2008 of the laboratory subgroup of the PMTCT Interagency Task Team (IATT), representing multiple international agencies, donors and implementing partners supporting PMTCT and early infant diagnosis. CDC-GAP is grateful for the careful review and support of multiple partners. The materials will remain in the public domain, as part of the expanding "toolbox"of guidelines and manuals being developed to support PMTCT internationally. These manuals can and should be modified and adapted to individual country programs, with modifications to reflect changes in technology, policy, and guidelines.
